Completed multiple Level 2 or Level 3 sessions successfully

No active acne, open lesions, or active skin infections in treatment area

Understand PIH risk — especially important for Fitzpatrick skin types IV–VI

Why the 12-pin cartridge again?

At Level 4 the 12-pin returns — not for depth alone, but for precision. Acne scars and stretch marks involve fibrotic tissue that must be mechanically disrupted. The 12-pin's concentrated needles penetrate scar tissue deeply and precisely. The 36-pin cannot achieve the depth or concentration required to remodel fibrotic collagen. Different tool, different job.

Why GF11 or Stayve PDRN Exosome?

The goal at Level 4 isn't just stimulating collagen — it's breaking down disorganized fibrotic collagen in scar tissue and replacing it with organized new collagen. GF11 provides intensive growth factor signaling for deep remodeling. Stayve PDRN Exosome combines cellular repair with exosome technology specifically for scar remodeling. The same serum used in treatment continues into aftercare — extending the biological signal during the open healing window.

Why up to 2.0 mm?

Acne scars, deep wrinkles, and stretch marks exist in the reticular dermis — the deeper structural layer. Reaching it requires 1.5–2.0 mm depending on the area. Depth is not increased uniformly: delicate areas like the eyes and nose stay at 0.25–0.5 mm. The increase is targeted only where scar or repair tissue exists.

Why every 4–6 weeks?

Scar tissue and stretch marks require collagen remodeling — breaking down disorganized fibers and replacing them with new, structured ones. That process takes longer than standard collagen induction. The 4–6 week minimum isn't optional at Level 4. It's not waiting time. It's when the actual repair is happening beneath the surface.

Every decision in this protocol has a specific biological reason. Here's why Level 4 is built the way it is.

Cartridge: 12-Pin

Beginner Max Depth: 1.5 mm

Advanced Max Depth: 2.0 mm

  • Acne Scars: 1.5–2.0 mm spot treatment

  • Forehead: 0.5–0.75 mm

  • Around Eyes: 0.25–0.5 mm

  • Nose: 0.25–0.5 mm

  • Cheeks: 1.0–2.0 mm

  • Around Lips: 0.25–0.5 mm

  • Chin: 1.0–1.5 mm

  • Stretch Marks: 1.5–2.0 mm

Speed: 5

3 controlled passes per area

- vertical, horizontal, diagonal or circular

Stop if skin becomes overly red or irritated

Every 4–6 weeks​

*Fitzpatrick IV-VI  users should consider more conservative depths due to increased PIH risk.​​

Before Treatment

  • Wash hands thoroughly

  • Cleanse skin

  • Disinfect treatment area

  • Use sterile cartridge 

  • Tie hair back

  • Apply numbing cream for deeper treatments

  • Remove numbing completely 

  • Disinfect skin with 70% rubbing alcohol

  • Dry skin

  • Allow alcohol to fully evaporate before beginning treatment

LEVEL 4 • REPAIR - Protocol

During Treatment - Active First → Slip Second → Needle → Reapply Active

  • ​Apply Active Serum

  • Apply Slip - Stayve HA

  • Work in sections:

    • 1. Vertical passes

    • 2. Horizontal passes

    • 3. Diagonal or Circular passes

  • Keep skin taut.

  • Do NOT drag the pen

  • Lift pen between sections

  • Add additional HA whenever glide decreases

  • Pat remaining Active Serum onto skin afterward

Endpoint

You want:

  • Pinkness / redness

  • Mild to moderate erythema

  • Even treatment response

  • Tiny, isolated dots — not streaming, pooling, or anything covering more than a pinpoint is acceptable

NOT:

  • Excessive bleeding

  • Scratching or tearing the skin

  • Long drag marks

  • Gray or overworked skin

  • Severe inflammation

 

Speed Settings

Lower speeds:

  • More control

  • Better for beginners

  • Better for delicate areas

Higher speeds:

  • Faster treatment

  • More aggressive

  • Better for thicker skin/scarring

💧 Glide Requirement (All Levels):

Always use a hydrating slip serum to minimize friction and protect the skin barrier during microneedling. Hyaluronic acid-based formulas are ideal—options like Stayve Medisco Hyaluronic Acid and Hyaron provide excellent glide and hydration support.

🚫 Avoid during microneedling:

  • Vitamin C (L-ascorbic acid)

  • Retinoids

  • Acids (AHA/BHA/PHA)

  • Fragrance

  • Essential oils

Vitamin C and Tretinoin during the treatment and recovery period.

Treatment day — 
flood skin with hydration immediately after. HA serums only. No actives.

PHASE 2

Repair

DAY 1
Barrier repair begins — gentle cleanse with Cetaphil, layer hydrating serums, no exfoliants.

PHASE 3

Heal

DAY 2-4
Allow active healing — ceramides seal moisture, no retinol or vitamin C, minimal routine.

PHASE 4

Barrier

DAY 4-5
Rebuild the skin barrier — ceramide-rich products, SPF every day without exception.

PHASE 5

Resume

DAY 6-7
Return to normal routine gradually — introduce actives slowly, SPF remains non-negotiable.
